Please complete the evaluation form and provide the necessary information prior to attending the scheduled player evaluation. This information will be used to order players Jersey for the Fall Ball 2024 Season.

If you are unsure of size, we recommend choosing a size up.

 

Please note that if top three jersey numbers are not listed, the league will choose on behalf of the player. Top Jersey numbers are not 100% guaranteed. Double check the information you have selected on the form, once jerseys have been ordered no changes will be accepted.
